Required Evidence and Procedure for Pradhan Mantri Janarogya Yojana/Ayushyamaan Card

Evidence required.

• No letter under Pradhan Mantri Ayushman Yojana
• Ration Card (New Barcoded)
• aadhar card
• 30 Rs. You can make card by paying per person.

• Security cover by Government of India up to Rs.5,00,000 per person per annum to each beneficiary.
• Based on the census of India in 2011, the economically backward families are covered under the
  
  
  Pradhan Mantri Janarogya Yojana.
• Thus, there is currently no provision to add names of new families if you or your family's name is not in the list.
• But, if you are living in Gujarat and have an annual family income of less than 4 lakhs, then you can get Amritam/Vatsalya card for treatment.
